About Our Project
The Problem
At present, 9% of the calls to the Welsh Ambulance Service are for people with a "Palliative" or life-limiting illness. [1] From recent research, paramedics and out-of-hours GPs do not regularly have access to medical health records. [2]

We know this can often lead to unnecessary hospital admissions.

A QR Code Wristband was suggested as a solution to enhance decision making.
​
This platform was found to be acceptable by:

Our Mission
Our project aims to ensure that people receiving care for a life-limiting illness are provided with both holistic care and safe care.

We aim to reflect all patients wishes utilising the QR Code Wristbands to have a person-centred record of their healthcare.

We hope that through good information sharing, patient care will be safer and more appropriate.

How is my data stored?
Your information is held on a secure professional server owned by a UK-based company that specialises in QR code wristbands.

The information can only be accessed by the company employees for administrative duties, yourself and the project team.
​​
​
Information from the server is not transferred to or from other systems. This way we know that your information is safe and available to help in your medical care.

Data Privacy: Your medical information will be kept strictly confidential and secure.
​​
​
Any data collected to help with this project will remain confidential, anonymised and discarded after 5 years.
​

Frequently Asked Questions:
1. Someone is unable to access the data on my QR wristband.
​
As with any online website, access to the internet is required – if the paramedics are unable to connect to the internet, please call your local proj

​​​2. My wristband breaks or is causing a sweat rash.
​
The wristbands are made of silicone and are strong, however if your wristband breaks please let your local project team know so a replacement can be issued.
If you develop a sweat rash, please contact your local team and they will be able to issue you with a fabric wristband. ​​

3. What if I lose my wristband?
​
Similarly to a bank card, if lost, please inform the project team who can freeze the webpage and help to supply a new wristband if you wish.
​
We can not guarantee that if lost that your data will be protected from others viewing your information so we would advise to contact us as soon as possible so we can help.

5. What if I want to change the information displayed on the QR code wristband website?
Please call your local project team members so we can best reflect the information you want displayed.

4. What if I become more unwell and am admitted to hospital or naturally deteriorate and unfortunately die?
​
If you become unwell and are admitted to hospital you can either wear the wristband in hospital or leave it at home and wear it when you are discharged.
If sadly you were to deteriorate and naturally die then, the project team will review your records to confirm this. We will then contact the QR code company and ensure that your medical information is deleted from their server.

6. I do not wish to continue participating.
​
You may withdraw from the project at any time. If you wish to withdraw, please let us know by telephone; do not have to provide a reason.
